Course Registration Form

1.  Type Course Name, start date and your parish in "Your Comments" section below.  Inclusion of your parish, is important because LAA will waive the course Registration/Book Fee ($20) for the first three registrants from your parish!  You must, however, include a $20 payment with your course Registration (as per paragraph 3 below) and a credit towards the tuition for said amount will appear on your course tuition bill if you are one of the first three registrants from your home parish.

2.  Complete Form below or send an e-mail to info@lourdesadultacademy.org that requests course enrollment and includes the information requested below.  Print and save a copy before submitting.

3.  Submit completed form and mail copy of completed form (or e-mail) with Registration/Book Fee of $20, (or $35 Late Registration Fee and full course tuition if registering within 5 business days of course start date), to Lourdes Adult Academy, One Eagle Rock Avenue, West Orange, NJ  07052.

4.  Your enrollment will be confirmed via mail, phone or e-mail upon receipt of your Course Registration Form (or e-mail request) and your initial payment.
